Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience:
Minimum 1 full year Adult Home Health Experience License: Must have
active license in hand Certifications: CPR, BLS Must-Have: Reliable
transportation, current driver's license, and agency-required
liability insurance. Description: Home visits for physical therapy
services. Periodic office meetings are required. Average number of
patients per day is 5-6. Contractor will be assigned between 5-10
patients per day. Patient visits and activities will have different
point values, totaling 30 points per week. No guaranteed hours; if
low patient census occurs, discuss floating to a nearby clinic for
hours. Types of cases include Start of Care and Follow Ups. Recent
Home Health experience is mandatory; first-time travelers will not
be accepted. Weekend requirements vary by location and profession
and should be discussed during the interview. Must be willing to
travel up to 75 miles from the home clinic. Mileage will be
reimbursed at the IRS standard rate. The commute to and from the
first and last patient is not billable. Local candidates are
accepted at the same rate. Pending licenses will not be accepted
unless only a few days are needed to obtain them. Travelers must be
open to taking any patient assigned, regardless of distance.
Documentation must be completed within 24 hours of the appointment.
There are no guaranteed hours with this contract; clinicians must
achieve weekly productivity points of 30. If not receiving enough
patients, the option to float to another clinic will be available.
